Service Request - - Report a Problem
Welcome! This web site enables you to request a public works maintenance service. Please note that this web site is for NON-EMERGENCY services only! For potential emergency situations, call 510-615-5566. There are five steps to submitting a service request:- Step 1 - Select the type of service request.
- Step 2 - Answer questions about the service request to help us better understand the request.
- Step 3 - Enter the location of the service request.
- Step 4 - Enter your contact information.
- Step 5 - Enter additional comments, verify the information you entered, and submit the service request.
Click on the "New Service Request" button to begin. If you prefer to call us to make a service request, we can be reached at 510-615-5566.
Existing Service Request
Welcome back! You can check on the status of a service request that you previously
submitted. You'll need the Service Request ID number and
the email address you used to submit the request.
Click on the "Check Status" button to begin.
